Insert your logo into a document and re-size it if necessary by clicking and dragging on one of the corner 'handles'. Make sure to use the corner handles and not the side or top/bottom 'handles' as this will distort your logo. On the Picture toolbar click on the Text Wrapping button and choose 'Through'. You will now be able to move your logo by clicking in the centre of it and dragging it.
Click away from the logo and from the Insert menu choose Picture-WordArt. Choose the style of WordArt you want and click OK. Over-type the words 'Your Text Here' with the address and phone number and click OK. You will be able to change the look, shape colour etc. of the WordArt from the WordArt toolbar if you want. Re-size it when you have the look you want, by clicking and dragging the 'handles', any of them can be used.
Move the WordArt where you want it in relation to the logo. With the WordArt still selected, (handles visible) hold the Shift button on your keyboard and click on the logo - both should now be selected. Right-click on them and choose Grouping - Group. They will now be one unit and can be moved or manipulated as one object.

If you already have the logo then, one way would be simply, Insert--> Picture as File...and go find the file and place it in the doc and then type yer address and phone number where you want it...

Another way would be to draw a two-column, two-row table where you want the logo, then, in the first column of the first row, place the logo, and in the first column of the second row, place your phone and address info, then go up and check table properties and make sure the table's border properties are turned invisible, and, if you wish, combine the right two columns so's you can type other stuff over there...and resize the two columns so's the content fits the column size.

Is that what you mean?

If you want to create a logo or something like that best thing to do is create it in photoshop and then add it as an image to any document you print. That way it is exactly the same every time.
