When "he" says "you r such a gem". It being used as a metaphor or a short way of saying something like "You r a diamond in a pile of gravel.
It's like saying you she is the most beautiful girl in the world.

Well think of everything a gem/jewel is and apply it to yourself:
precious
georgeous
rare
special
beautiful
invaluable
etc... [ BTW... they are all good adjectives...if a man said this to you, you are a lucky girl!]

it is a typo, it really means "you are a germ." Try to wash more carefully
