i want to cancel my gym membership?

I'm currently with Goodlife and I want to cancel my membership for 2 reasons. First, I haven't used my membership in almost 9 months. Second, I was in a car accident and the gym that my insurance company wants me to go to is much closer to my house.

I've already been a member at Goodlife for1 year, so now I'm on a month-to-month basis with them. What information do I need to put in the letter they require in the event of cancellation and where can I find a template of such a letter?

Have you checked their website or contacted them or even called the gym to see if you need a standardized letter?
