Usually, it's hardware failure. It could be because of broken hardware, but more commonly it's buggy drivers, especially the video card. Sometimes the video card stops refreshing, making it look like the computer is frozen, but in reality the monitor has stopped drawing images.

When too many programs are running thus taking a lot of CPU usage. Especially when a virus or other scan is scanning.
