The biggest problem may be your commitment and interest. Have you thought about why you "want" to study? It really sounds to me as though you want to do anything but study. If you can take control of yourself, I suggest that you make a schedule of what you are going to do to study. Write down a list of specific things you want to complete, such as "Read pages 20-35", "Answer questions 1-5 on page 36", "Review and type class notes from Wednesday", "Test myself on vocabulary terms from chapter 6". Make an estimate of how long the tasks should take and choose a starting time for each task. Check off each successfully completed task when it is done. When you fail to complete a task according to the schedule, think about what happened to prevent it, and write this down on the list. Then re-schedule the task. Assign yourself a study time for each day of the week. If you can, it may be helpful to study at the same time each day. This will help study activities to become habitual. Good luck.

make sure you have all youre matter in front of you. make sure all your notes, text book references, etc., are compiled into one file, or maybe even try to copy down all the important bits into one single journal. that way, you dont have to run around the place looking for stuff (its deadly boring, and hell, you lose your tempo). sit down, make sure you are comfortable. study lying down on the bed if yu have to. make yourself a drink or a snack.
read through the matter a couple of times. dont try to rote learn anything, dont sit and memorise any dates, NOTHING. just keep reading through that fat book of notes over and over till you get an idea of what youre supposed to know. do the important bits over. quiz yourself at the end of each session. if you think you can elaborate on something, or cut down on something that is too elaborate, keep space on the sides for your own notes.
i studied like this all year, and i did fine!

I try to make it fun.
I read slowly and try to understand it, i talk to myself often when i study, like there are two people there, discussing what I just learnt
they go like "ooooooh, this connects to that other thing on the last page"
"but it's so gross"
"no, i actually think it's kinda cool"
though that might not work for math very well...

Relax, and take deep breaths. Fight your mind, "I must do this, I must do this!!" If that doesn't works, drink a cup of water and FOCUS!!!! Don't think of anything. Do something that motivates you!!
