As long as the provider you are purchasing from ships to the US there should be no problem. Just put the child's name and address in the ship to section when you buy it.

It depends upon what you are buying. In many cases you can ship to an address differently than the credit card address so it shouldn't be a problem

just go to a US website and make your purchase using your UK credit card. it will be charged in US dollars, converted on your account, probably with a foreign exchange service charge in proportion to your charge.
if you go to a UK site and purchase a gift, you will pay overseas shipping and taxes even.
