what does this '12/11/12' , '11h12' and 'hammer on' on acoustic guitar tabs???

12/11/12 --- put finger on 12th fret, strum with the pick, then without strumming again, slide fret finger down to 11th fret then back to 12th fret, quickly. listen to the song to hear the difference if you have trouble (this is a slide)

11h12 --- 1st finger on 11th fret, strum note. without struming again place 2nd finger on 12th fret right underneath your first finger so you now hear a different note without using your strumming hand. (this is a hammer-on)
