OK - this one's easy! It's a tie between the nameless narrator (aka - "the underground man") in Dostoevsky's "Notes From The Underground" and Zarathustra – Nietzsche’s despicable anti-prophet in "Thus Spake Zarathustra"
I think both were portrayed superbly by their authors; they are so good at being despicable that you love to hate them!!! grrrr!
Oh .. oh oh ... and Ignatius J. Reilly in John Kennedy Toole's "A Confederacy of Dunces" I guess that would make it a three-way tie

John Ryan Patrick jr, from Tom Clancy's Teeth of the Tiger. This is a character invested with none of the personality of the original John Ryan, whom made Clancy so famous. The only good news, is that I have not seen a new novel out from him in a while, so he can't further ruin a formerly great series.
*p.s. with the other limitation you posted, can Harry Potter books really qualify as literature, as there are many examples of far better storytelling in comics than those books? They are great examples of the power of marketing, but little else.

I cannot think of any character that I can say I hate. I don't like their actions, but when I think of it, I can usually draw upon some sympathy for them. Maybe I choose to forget the people I "dislike intensely" and only remember the people I like or have sympathy for. I can only remember people I "dislike intensely" who were or are real, such as Hitler. I suppose I also disliked Voldemort for some of the same reasons.

Dr Chilton in "Red Dragon" and "Silence of the Lambs".
a good "bad guy" is often the catylist in a great movie. Alan Rickert in "Die Hard" was awesome---it had never occurred to me that a Brit could do an "American' accent. he is Snape in the "Potter" movies and he is the voice of God in "Dogma"
