Yes, stars are still being formed in space, and they will keep on forming, until most of the Hydrogen in the Universe is used up, at that point, the Universe will start to darken as each star uses up it's Hydrogen and dies.
Way for in the future the Universe will be dark, with no star light at all.
But don't worry about it, for every one in the Universe will either be dead, or found a way to go to another Universe, that is if there are really other Universes, like some believe.

Yes, and the early stages of a star formation can be observed. This initial process is called a protostar. The stars are formed when nebular matter grows by the gravitational action to a size and internal pressure enough for the nuclear reactions be started.

Please take a look at

Well the answer is yes, the stars are all forming from the very big clouds mostly made of hydrogen. After millions or billions of shining(depends on their mass) they will die. some of them have a very violent process of diying in a very big explosion that can outshine an entire galaxy. And these explosions make other clouds of dust and hydrogen and other heavier elements, and from one of these types of clouds our star(sun) and we ourselves have arised. This process is happening all the time until the they that no star explode`s and no hydrogen remains in the univers
