Cracked head = how knows? It just happens over time. My guess is quality of coolant. One known cause is mechanical failure. Head bolt break and is bounced around the head while engine runs. And it happen to get between the rocker arm and valve.
Blown head gasket = engine overheating, causes the cylinder head to warp then head gasket seal breaks and leaks oil into coolant or coolant into oil hole.

some cars are notorious for blowing head gaskets, its not always something you can prevent, its a common problem, you probably didn't do anything, some cars that use the orange antifreeze blow the head gaskets because the antifreeze eats at it

Generally by over-heating, caused by a low engine or radiator water level at one time.

well, when the car over heats for a long time that could cause it. however, driving without coolent could do it, a bad radiator or a non working radiator fan also could be a reason.
