I want to help a homeless woman, but she believes "help" equals taking advantage.

Probably not, but you can explain to her that as a fellow human you enjoy helping others and your rewarded by the act; she has no obligation but to take the act of kindness. The poor thing probably needs meds and without them cannot grasp someone who just has a good heart.

yes you can help ... and goodonya! its great , if she is worried about taking advantage of you ... get her to do things for you or for other people or whatever she is good at , her skills maybe very useful to someone and when she see's that the need for her talents and the need for her as a human being is back she won't feel like she is getting something for nothing and she CAN give something back ... wish you all the best as it won't be easy ...but you can make a difference and being nice and thinking about others ... costs you nothing and you DO get so much more back ...
p.s. no I am not religious in anyway ...just a fellow human

You know first thing I thought when heard word "Paranoid"
is no you can't. But if think about such people as persons
who don't know who to trust situation seems to be in another light. And as long as you want her to trust you, you should trust her first . without your trust to her it will not work. And here is at least two persons opinion, who are thinking about getting some use of her :) and in this light I don't think that her state of mind is absolutely abnormal.
Besides right now I'm thinking about word " Paranoid " as abuse.

yea and don't "play games" with her like in first advise.

In this case:think up something she could do for you or somebody else, then tell her this is not free but she did something for the help. This always working with pride people. And you do a good think.

Maybe you can think of a way to help her without her knowing that it is you that is helping her.

yes,you have to gain their trust . once you gain their trust , they will let you help them
