Umbridge handed Malfoy Harry's wand before he went to get Snape for her, which was before she, Harry, and Hermione went into the Forbidden Forest. Ron handed Harry and Hermione their wands after Grawp and the centaurs deal with Umbridge and he, Ginny, Neville, and Luna catch up to the two. He, Ginny, Neville, and Luna had managed to turn the tables on the inquisitorial squad after Umbredige left with Harry and Hermione and they retrieved their wands for them.

Ron, Ginny, Neville and Luna were left with the Inquisitorial Squad. They were able to escape them and get Harry and Hermione's wand from them. So they were able to meet them at the Forbidden Forest at the time when the Centaurs took Umbridge and they gave Harry and Hermione their respective wands before they set off for the Ministry of Magic.

Haven't got the book here now but I think that Umbridge leaves it (and the other captives) in the care of Draco and his sidekicks... who are distracted and overcome by Ron Weasley and the others. Ron then brings the wand to Harry.
