It happens during labor and birth if the uterus is torn and an artery is opened if the tearing is deep enough and there is not muscle tone to slow the heavy hemorrhaging.
It is very rare to die in childbirth in the 1st world countries or even in many 3rd world countries but it still happens everyday usually in very poor countries where there is not medical treatmeant.

1] the uterus is a muscle. Sometimes after delivery, the uterus is too 'fatigued' to contract, so all the blood vessels that connected to the baby and the placenta are wide open, pouring out the blood. [ You can actually hear it running on the floor! ]
2] in this country, we routinely give moms pitocin after delivery of the placenta, to prevent this in the first place.
3] if by some chance, anyone reading this ever has someone deliver a baby and bleed, put one hand at top of pubic bone, and press down to back [ keeps uterus in the abdomen ] ; and use second hand to rub the top of the uterus [ right about belly button ] to make it contract.
4] the episiotomy, or the tearing instead of episiotomy, will bleed freely [ as if you cut an arm or leg ], but does not usually put you in danger of bleeding to death - in comparison, it hardly bleeds at all.

Realizing a human being is coming out of their vadge and they die of shock. Sounds pretty scary to me! But really I think it's hemmorage or just excessive bleeding. Usually just something with the blood generally. I heard of one lady that had an anneurism after giving birth, I think it's cause you push so hard.

First of all, Ashley it would be spelled hole.
Second of all, epecially in countries such as the US, etc. this doesn't really happen anymore. Unless there is an undiagnosed blood issue, you'd just transfusions. But you can lose a lot of blood when giving birth and it can happen over a long period of time.
