It's used as an insult. A moronic pronunciation of 'W', his middle name.
To my shame, I cannot think of a similar term used in the United Kingdom...
I suppose you could look upon the press usage of Bliar when they spoke about Tony Blair - the deliberate transposition of the 'a' and 'i' to criticise Tony Blair for being a Liar over the 45 minute 'sexed up' intelligence report on Iraq's WMDs.

Because he is named after his father, people would use initials to identify which one they were talking about:
"George Bush sucks!"
"Which one?"
"George W Bush, not George H. W. Bush"
Eventually, people just said the "W" to indicate which one, and then just used the "W" for a nickname. The Dubya spell-out is to remind people that he is jest a good-ole -boy, and is part of the way his far-right cabal marketed him for election as their front-man, because after all. who would have elected Cheney, Wolfowitz, Pearle, or Rumsfeld if one of them actually ran for election?
