I have lost my passport, old & just newly received passport, no idea how, i am unable to recollect ,I think i lost it at home since it was not neccesary to carry it outside the house.. I may be accidently lost in the garbage. how or where do i give complaint in USA, before applying for new [passport, what all i should expect when i go to register complaint, any suggestion would be helpful

To Report a Lost or Stolen Passport:
Complete Statement Regarding Lost or Stolen Passport, Form DS-64

Fill in as much of the passport information as you can.
Answer all the other questions in detail.
Sign and submit the Form DS-64, Statement Regarding a Lost or Stolen Passport, to:

U.S. Department of State
Passport Services
Consular Lost/Stolen Passport Section
1111 19th Street, NW, Suite 500
Washington, DC 20036

Do not mail your new passport application to this address. In order to obtain a new passport you must appear in person at a Passport Agency or Acceptance Facility.

Operators are available 7 a.m. to midnight, ET, Monday-Friday, excluding Federal holidays.

Go to http://travel.state.gov/passport/lost/lost_849.html and follow their directions. Or, go to one of those post offices that process passports. The website will probably be less hassle; I never have good luck in person.

my ex wife threw my passport awy
call your congressman first repor it
then you have a letter to the house
then ou go to the library or post office and new form
you will get a new one with a designation lost or destroyed.

can I have a BA please

Call your consulate so that they can reissue you a new one.
