www.rideaccidents.com has lots of great info on amusement park accidents, including deaths, as well as investigation information.

People die at all amusement parks all over the world every year. Considering the millions of people who go, I'm surprised such a small number or injuries and death occur. Many of the incidents are due to the guests being stupid and/or ignoring warning signs and going where they aren't supposed to, though some are due to maintenance issues.

There is a really good page at Snopes.com, it lists all 9 fatalities at Disneyland since 1955.
