It originated from some video game forum, when someone posted what was supposedly a link to a trailer for Grand Theft Auto IV, when in reality it was a Rick Astley music video.

Well, chances are that it came from 4Chan and then the GD (Gaia) copied it and made it mainstream. 8)

And as for where the name came from, His name is Rick, it's called RickRoll'd.
