i would just say you are going to be in the hospital for a couple weeks and ask what kind of documentation do you need . usually they have a nurse and you would get a statement/form from her to fill out. they will process getting your disability paid out since you willl probably be entitled to benefits. your boss does not need to know the name of the hospital or your diagnosis.

sorry, but ive been in a mental hospital for the past 12 months... im only 17... its not fun and they're hard to get into.
i was meant to be in hospital again now, i was taken there, escorted by 2 police officers, but they wouldnt take me...
go figure... but yes... you'll need proof of some description.
