Usually a pitcher will throw to bench players from their own team and will pitch about 100 pitches. This gets them used to facing Live batters without having to go to AAA for a rehab start. You see this mostly when a team is on a road trip and far away from the farm club or when the manager wants to monitor the pitcher's progress.

They go through the motions of an actual game. Usually about 80-100 pitches plus a little practice fielding, running to cover 1st, etc. This is done to make sure that they have recovered enough from their injuries to do it for real.
