Hi , I want to change my middle name because it is Andrew , and I don't like it , I want to change it to a Polish name. 13 is too young.. your parents/guardians would have to change it for you. (unless you become an emancipated minor.. but I am 99% sure you are still too young for that even.)

at 18 you just have a good reason, pick up the papers from your local court house.. fill them out (or pay a lawyer to do it for you) and go before the judge.. tell him your reason and it's done.

Step two: Let the lawyer take care of everything.
Step three: Enjoy your new middle name.
