Water pollution is when a large ammount of chemicals, or hard metals are found in water. Fish die from chemical pollution because, depending on the chemical, it can clog or harm a fish's gills or organs.
Fish die from hard metal pollution because a large ammount of dissolved hard metals stops dissolved oxygen flow, which suffocates the fish.

Fishes die in polluted water because they don't have enough oxygen. It's quite similar to how a human would die if they were in a room that didn't have enough oxygen for them to maintain their body.
