ICANN determines the top-level domains (TLDs) such as .com, .net, .uk, .gov, etc. You cannot make up TLDs, only ICANN does so.

ICANN then licenses registrars (companies) who can sell you second-level domains -- yahoo, google, godaddy, etc. -- within a given TLD. Often, there are many registrars who are permitted to sell SLDs in a given TLD -- for example, Yahoo, GoDaddy, Network Solutions and Register.com can all sell you a .com, .net or .org name. But only a few companies -- such as register.ca -- can sell you a .ca (Canada) domain name.

If you wanted to create a new TLD, you would start by petitioning ICANN, which would then present the idea to its committees and staff for review. Depending on their review, public input and member feedback, the board would decide whether to create the TLD.

But even if it were created, the fact that you suggested it wouldn't ensure you get first dibs on using it. All TLDs are open to use through any of the approved registrars.

So your best hope would be to get only one registrar approved to sell your new TLD -- and the chances of that are about zero, since there's huge money to be made any time a new TLD comes out, and all the registrars would spend big money to fight a single-registrar TLD that wasn't tied to a specific country (such as .uk, .ca, etc.)

No you can't arbitrarily create Top Level Domains. You must purchase your domain from a registrar.
www.godaddy.com www.networksolutions.com etc

We purchase our domains from the Domain Kings, they make the rules, they decide what to use. You cannot just "Create" a domain ending in .kom

It's called a TLD (Top-Level Domain)

check out one of the registrars mentioned above, there are plenty there, many of which you've probably never heard of.

if you cant find what you want there you can try convincing ICANN (http://icann.org) to add your TLD :)

Sorry, you can't make up your own. Domain names have to be purchased, and the suffixes are fixed. (In fact, they're actually supposed to mean something, like "commercial", or a country.)

Here's a list of the top-level domains, pick one of those (if you qualify): http://www.computerhope.com/jargon/num/domains.htm
