I take this to mean that a true friend will tell you something to your face even if it will hurt you as long it is for your own good in the end.

It means that even if they do something really hurtful they don't try and hide it because they are your real friend and care about how you feel. A deceitful, sneaky, and unloyal "friend" would stab you in the back and not think nothing about it.

I am going through something like that myself at this time and can tell you that I thought I had a true friend, but appearantly she didn't think so, meaning that I never had a true friend.

It means that if they are going to betray you, they wont be sneaky about it. They will come to you and state their intentions straight up out of respect for you. (say for example they say "I like your boyfriend and chances are im probally going to make a play for him", as apposed to a chick who hides it all.
Its basicly a sign of respect
