I think there is an American distribution center in Spokane, Washington or nearby in that part of Washington state. I assume you bought something from them directly and it's been taking a long time to get to place. I am sure Apple has distributors all over the US.

Yes if your state has a sales tax and apple, or best buy, has a retail outlet or any other facility in your state which they probably do.

i think its china, i know the computer come from there, im not sure about the accessories though.
