Yes, anything thrown from the rubber to home plate is considered a pitch!

Yes, they're included in the pitch count.
They have to be, especially since there's nothing stopping a batter from swinging at the pitch, and there's also always a chance that a wild pitch will result.

Yes. And when they are thrown, they are counted as balls.
