Are you looking for a handicapped placard for parking? The same office where you get your license plates for your car issues those. You need to ask your doctor to fill out and sign the form for it, then turn the form in at that office. You can pick up the form at the license plate office, or, many doctors keep them on hand in their office. You MUST have the form signed by your doctor to get a handicapped placard.
