Belt was replaced but looked good. Upper idler was replaced and belt tensioner is good. After starting engine two inside groves on lower left idler are showing. Bearing in idler are good and spin free. Could idler have been installed incorrectly?

I know there should be no groves showing. After putting the belt on correctly the belt moves out 2 groves.

The tensioner is likely bad. Had this happen with my '94 LeBaron. Everything looked and felt great, but looking very carefully at the tensioner, it was actually leaning slightly. Replaced the tensioner ($70.00) last year, and I've had no further problems.

Change the belt tensioner. You cannot tell if it is good by looking at it.
Make sure all pulleys are aligned.

Idler pulley. The rattling you here is that. It starts wobbling and shacking while spinning then whoopee!! the belt fly's off. The shredding comes because the belt is ripped around the rest of the pulleys.

I've ran into this on trucks at the shop, check your alignment of all the pulleys & also check the new tentioner to make sure it's the correct one & make sure the tentioner is installed correctly.

You answered you own question. There should be no grooves showing when the belt is properly installed. It should fit from end to end.

It is the wrong pulley. It should line up exactly.
