It implies that someone is a secret agent, and that the secret is so important that the person would have to kill you in order for the secret not to be revealed. People like to imply that they are secret agents, so this has become something of a joke when someone asks you to tell a secret.

If I tell you a secret, I know you will tell someone else. So, if I tell you, I will have to kill you to prevent you from passing it on. What a cheery thought.

It means it's a secret and if you told them the only way you'd be the only one to know is if they were dead.... so once I told you I'd have to kill you to keep my secret.

It's a cliche used when you wish not to divulge something but don't just want to say I'm not going to tell you.
