Just a fleeting thought...like the most serious incident I've heard of was Casper Van Dien broke his index finger or somet while shooting the 3 way axe fight in Sleepy Hollow...but has anything worse ever happened to anyone?!

Actors have been killed while filming, notably:
Brandon Lee - Shot and killed by a gun that should have had blanks during The Crow.
Steve Irwin - Worked with crocodiles, but stung to death by a stingray while filming Ocean's Deadliest.
Vic Morrow - Decapitated by a helicopter while filming Twilight Zone: The movie
Karl Wallenda - Founder of "The Flying Wallendas" killed in a trapeze accident, but not being filmed for any production.
Probably more, I knew about Brandon Lee and Vic Morrow, but search Wikipedia briefly.

Christopher Reeve didn't injure himself doing stunts for a movie.
-Brandon Lee, was shot by what was supposed to be a blank, and died while filming The Crow.
-Vic Morrow was filming a scene for The Twilight Zone, when the stunt helicopter crashed on him and the two children in the scene. No survivors.

yes, there have been many instances of actors being injured on set. Keanu Reeves was injured filming the original Matrix, and Vic Morrow died during filming of Twilight Zone, The Movie, doing a stunt he shouldn't have been involved with.

The actor Vic Morrow was killed while filming the movie, "the twilight zone." He was killed during a sequence where he was escaping it a helicopter crash with seven children. Some of the children were killed also. They left it in the movie so you can actually see his death.
The other actor that comes to mind is Brandon Lee being shot to death on the set of the film, "the crow."

Not really an actor, but Richard Hammond almost died while filming for Top Gear when he crashed the jet car thingy

Dar Robinson, "The Ultimate Stuntman", who never broke a bone in his body in his 19-year career, died in a motorcycle accident on the set of the movie "Million Dollar Mystery". Check out this link.

bam margera from Jackass hurt his c*** while filming Jackass when he was doing a stunt. They put the xray of his c*** in the movie. There has been things like sprained ankles and fractures
