You have to remove the concrete - a lawn requires a well prepared plot of ground to do well.

You can't- you need to gety rid of the concrete first. The reason why the grass is yellow is because it is deprived of nutrients. How would you like a diet of concrete?
Give grass soil and water, and it will grow!!

break up, then pick up the old concrete... fill the holes with good soil, water it down really good... then go back and fill in where the water made the soil sink down to unlevel..... sow grass seed... keep the area moist, not soggy until you see the grass sprouting.... wait until it's more than three inches high to mow it.... keep it watered and moist until fall.... it would be best to wait to do this all when it's cooler, anyways.... grass wont' do well in this heat right now.....
