y = 5x - 8
5x - y = 8
Question 2
y = (-5/3) x + 12
3y = - 5x + 36
5x + 3y = 36
Question 3
y = 18 + 3x
3x - y = - 18

Just remember that f(x)=y. So,

y=5x-8 or 5x-y=8
y=-5/3x+12 or 5/3x+y=12
y=18+3x or 3x-y=-18.

The column on the left is just 1, 2 and 3 respectively, with y=f(x). The right column is just the left but the constant is isolated on one side.
