you know anything you build needs a sound strategy.Much thinking and tests .Now what you think will happen to your design if such is not applied thoroughly. You may explain this to a kid he /she will find the answer fast!

No,it was caused by idiocy and incompetence.In 1990,it was proven that repair was needed to that bridge.That repair never happened.
