I can remember watching the buffalo sabers game on T.V. when Clint Malarchuk got his jugular vein sliced. I can remember when everyone noticed what happen to him, both teams where going nuts on the ice calling for anyone that would listen. When Clint took his helmet off he took his hand of his jugular vein and it looked like a water fountain, every time his heart would pump, a ton of blood would poor out. I will never forget that game!

Another incident that comes to mind, I didn't see it, but I heard about it. Was when Borje Salming from the Toronto Maple Leafs was lying on the ice and a player from the other team stepped on his face, about a couple of cm from his eye and pushed off. Lucky enough, he didn't loose his eye, but he had a ton of stiches.

First off, great question.
Second, gotta love tblightning's answer.
Clint Malarchuk's is definitely one of the most unbareable NHL clip to watch. It sends chills through my body because it can happen to anyone. How about Chris Drury getting cheap shotted by Chris Neil. It wasnt as hard to watch but it definitely didnt do any good for the NHL. Claude Lemieux's hit from behind was also pretty bad as long with the slashes to the face which really is uncalled for. Bertuzzi is a good player but he really wasnt thinking when he did his little incident.
This clip will list some nominees:

I'm sure most fans have seen these though
What about Philadelphia vs. Montreal Pregame Brawl 1987 on youtube. Its not like somebody getting hurt, but it wasnt something that was pretty.

I would have to say Clint Malarchuk even though I wasn't alive back then, but I've seen it on youtube.
I would have to say Jiri Fischer with his heart stopping on November 21, 2005 against the Nashville Predators. Fisch was having a career year and now his future is in question. At least Malarchuk was able to come back. The same cannot be said for Fischer:(

On TV, a few incidents come to mind:
1. Clint Malarchuk's jugular vein sliced
2. Claude Lemieux drilling Kris Draper face first into the boards
3. Marco Sturm slammed into the boards
4. Kevin Stevens being knocked out in mid-air, landing face first on the ice
Worst I ever saw while at a game: Pat Peake's career-ending foot injury, where he went full-speed, skates-first into the boards in the 1996 playoffs. Doctors and surgeons had said that an injury that bad normally only happens to construction workers who fall off very high platforms and highrises; his heel wasn't broken, it was shattered.

For me it was the Malarchuk incident. I was going to school in western, NY and was in the Aud for that game. I was at the top of the first level near center ice and you could see every squirt of blood hit the ice. I had to turn my head because quite honestly, I didn't have the stomach to see that much blood. I heard people in the crowd say that he must have hit his jugular because of the way it was coming out. The trainer literally saved his life by putting his hand into his neck and pinching off the vein until they started stitching him up at the hospital.

I only caught the Malarchuk and Bertuzzi incidents on video replay but I did watch Draper get cheap shotted by Lemieux. It was horrid, but if I had to vote on any situation I'd give it to Malarchuk.

I watched the Clint Malarchuk game live.. nearly lost my lunch.. as horrific as Jiri Fischer's was (we learned later how close to death he was) it was bloodless.. Malarchuk there was so much blood on the ice.. I still couldn;t believe he skated off the ice.. He was never the same after that.. finsihing his promising career in the IHL in Las Vegas..

I was at the Malarchuk game, and you are absolutely correct. It doesn't get any worst than that.
I also was at the following game, when Malarchuk was introduced to the crowd from the Zamboni entrance. That may have been the longest ovation I've ever heard.

Well, I haven't been watching hockey for too long, so I'd say the Tucker hit on Sami Kapenen last time the Leafs were in the playoffs.
