There are some terrifying villains in the movies from both past and present, but the ones that stand out to me are the villains that look human, but have some feature that makes them evil looking. The only one that pops to mind right now is Colm Feore's character from "Storm of the Century." He looks very normal, but his eyes give him that evil look. Especially when they turn red.
As for terrifying looking in general apperance, Tim Curry did a fabulous job as Pennywise.

Even though it isn't a hollywood movie...
...Pan's Labyrinth has the scariest villain of all...The Pale Man.
Or you could just go with Norman Bates.

the scariest villain or i should say villainess is the head witch from the movie, "the witches". this movie came out in 1990. Definitely check this movie out.
