I mean, what's wrong with your thumb?

A shoehorn is used by a shoe salesman to assist in getting a shoe onto a customer's foot. If you think about trying to insert your thumb into the back of a shoe you're putting on a customer seated in front of you, you'll quickly see that there is no safe (or even logical) way to use a thumb!
Shoehorns have been around since at least the fourteenth century. In those days, men wore stockings that allowed them to show off their calves to women, and women wore large-bottomed gowns supported by hoops and undergarments. The men's stockings were very tight, preventing them from bending their lower legs at the knees, and the women's gowns tended to rise high into the air whenever they bent at the waist. As a result, neither sex was able to put their shoes on while seated unless they had servants to help them.
Long-handled shoehorns were invented to allow either sex to put their shoes on while seated. Just look at some of the period clothing to see the problems their clothing offered.

That is a really odd question for the astronomy and space section but actually give them time and NASA will no doubt invent a $14,000.00 per copy improvement for use on the International Space Station. I'm sure there is nothing more annoying than doing a 360 midair flip while putting on a pair of Manolo B.s in zero G.

Nothing, but if humans can find or invent some gadget to make things easier they will.
And using your thumb can hurt if the shoe is particularly hard.
