My father just received a form for proof of claim again NovaStar Inc., which is under a class action lawsuit. Is it mandatory to submit the information we are being asked for or can we ignore it if we do not want to be part of it?

No, you do not have to submit anything, but you will not be able to collect anything from the judgment if you do.

Doesn't sound like a class-action suit to me ... a proof of claim is usually just for bankruptcy cases, it might be some case that you're already part of and will get a settlement check from.

Re-read the document and contact the office of the attorney who filed it.

You can ignore it if you don't want to participate in the class action suit.
