If it's only 20, it can still live for a long time yet. I would agree with a diagnosis of a swim bladder problem, however. If the fish is still eating, try feeding him some cooked green peas with the skins removed. Sometimes they eat too quickly (especially dry, floating food), and this expands and releases gasses inside the fish. The peas are "fiber" to help push the food out.

If Bob is usually fed floating foods, soak them in a little container with some pond water for about a minute before he is fed from now on.

If this doesn't help him, he may have a condition called flipover which does need veterinary care.

Loss of balance when swimming is likely to be associated with a problem in the swim bladder. This can be reolved depending on the exact underlying cause. I recommend you seek specialist veterinary attention (Yes, it exist!!) and try to sort it out.

You probably mean Koi and if it is swimming on it's side, I'd say old age has caught up with it. Sorry. Tell them to make sure the water is WEL AIRATED!

20 years is a fair age and I'd say the old feller is on his way out.
