* Complete an Application For A Social Security Card (Form SS-5); and
* Show documents proving:
* Age and Identity.
* Take (or mail) your completed application and documents to your local Social Security office.

Your local Social Security Office will replace it for you.
Make sure you have Identification with you.
Call Directory Assistance or look up the number in the Telephone book.
