Usually, you can only roll-over 401k funds into an IRA and not the other way around because your employer pays certain administrative fees to maintain a 401k and won't want to pay additional fees for your own retirement funds generated outside of direct employment contributions from them.

Usually this can be done. The only reason(that I can think of) to do so is to take advantage of the ability to borrow against the balance in the 401(k). But you pay a huge price in loosing flexibility in what you can invest in. The 401(k) also is usually less desirable for your heirs if you die before you move it back to an IRA.
Unless you absolutely have to borrow the funds, leave it in an IRA.

yes, as long as they are pre-tax dollars but you want to check with your employer to see if their Plan Adoption Agreement will allow it. you will also have limited options of what you are able to invest in. why not leave it where it is (as long as it is not an IRA CD)? if it is an IRA with the Trust department of your local bank then you can invest the monies almost any way you'd like. eg: mutual funds, modeled accounts (diversified for you), individual stocks.
