Practice this in front of the mirror. Smile and say " Ohhhhh, it's so cute. You shouldn't have." When you do get it, never roll your eyes or laugh so hard.
I've done this a couple of times. I tend to gauge the person and the company I am with, the time of the receipt. If they're okay and can handle it, I would joke "what were you thinking?" and laugh it off. Or just smile and suck it it.
No matter how hideous it is. It's the thought that counts.

You first think to yourself that someone cared enough to spend a lot of time hand-making something for you and by-God you're going to act grateful. Then you say "Oh thank you! You must have spent so much time on this just for me."
Then hopefully you'll treasure it for what it is, an expression of love that took far more than money - it took time and effort. If you don't appreciate it now, maybe you will when you grow up and realize that he/she didn't have to give you anything.

I'd say something like "did you make this yourself? it must be great to be able to do stuff like that" That way you show interest and give praise without having to lie about it. Then, when they were gone, wash it on a really hot wash to shrink it to the size for a dog and give it to someone with a little pooch! Dog owners who put their dogs in clothes have no taste anyway so they'd probably love it!!

Thank you.
The later when the person is gone I stuff the thing in the back of my closet to never wear it. Then later in the year I throw it away without telling the person. If she or he persists in me wearing it I make the exuse that I lost the thing and don't have the motavation to look for it.

Well, what you should do is refuse to open gifts in front of people. And if you hate it, tell them you like it just fine, but don't over do it. If you do it right, you'll get away with only wearing it once and not hurting the persons feelings.

I would accept the gift gratefully, and thank the giver. There is no call for hurting other people's feelings, because they have bad taste in gift giving.

You have to be polite. So I would say thank you, and then when I got home put it in the bottom of a drawer.
