This is usually given on flights arriving in the Philippines before landing.
This is where you state your name, passport number, age, purpose of stay... etc.
I need this for my friend travelling to Manila, he wants me to show him exactly what to write there.

i looked but they don't have it. Just tell your friend not to worry it's pretty basic and stewardess will actually give them instruction on how to do it and your friend can always asked the stewardess question on what to put in the form.

Sorry dude! The Philippine immigration bureau website is down. And I doubt if there's an available form online. Don't worry. The form is quite easy to fill up. As long as your friend can read and follow instructions, he'd do fine.

It's not available online. As long as your friend understands instructions in English, he can answer the questions. It's just basic stuff like name, where he's staying in the Philippines, reasons for going to the Philippines, etc.
