In a spouse? Physical hurt, but only by a smidge. The rest of those fall into the unacceptable column.

one single physical hurt because
betrayal you can hardly forgive because you dont trust them anymore, abandonment you cant really get over someone that has left you for a long period of time, ongoing put downs destroy your confidence and makes you resent the person

I would easily forgive one single physical hurt, but the others are unforgivable, in my opinion.
