A diamond necklace ($1500.00!) that my husband gave me.
I was taking it back to the store to get the chain shortened and my grandmother insisted i wrap it in tissue. I forgot about it being in the tissue and in cleaning out my purse, threw it away.
It was 2 days before I remembered where it could have been so too late to find. And because it was so new it wasn't insured yet.
I could have claimed it on my homeowners policy i think, but that was years ago.

I received a statement in the mail that said my insurance company was going to pay for my son's care after he was born. It was $3600 that I didn't have to pay.

Meanwhile, having received that statement, I was under the impression that my son was successfully added to my insurance. I completed a form online and hit submit. I later learned that my session had timed out and he was never on my insurance.

The hospital then sent me a bill for the $3600. I went to the insurance company & they said he wasn't insured and I was past the grace period to add him. They said I could file an appeal, including all documentation that might help my case. The statement that said they paid for his hospital care... I must have thrown it away at some point. That cost me $3600 + several months paying out-of-pocket for his doctor visits.

My father's new credit card. Okay. Here's what happened. I'm distracted by my very busy toddler. My five year old was talking to my dad about SOMETHING. My dad hands me a credit card and says, "Get rid of the old card when you have a minute." So I cut the card he handed me and put it in the garbage.

NOT MY FAULT! He didnt' say, "Here is the new card. Could you get the old card and then cut it up and throw it out?" THAT I would have gotten!
