I think it is because it will turn it red and burn it.

What is the real reason? Anyone know?

Because it will cause more rapid cooling of the skin and could cause frostbite and tissue damage/death. Hospitals tell you to use ice because it helps swelling, but they don't tell you to put it directly on your skin.
The amount of time it takes to develop frostbite depends on the blood circulation to the area and other factors regarding your health.

Well, if you're talking about the drug, well then, it will burn your skin and leave you looking hideous. But you're talking about the other ice and the reason is, it cools your skin and can mess with your circulation. You can put it on your skin if you can handle the coldness though and if you can handle staring down at the frostbites which soon appear.

To answer your second question, well it takes approximately 10 minutes for frostbites to appear if your skin is in contact with something at or below 0° C. And you are told to use ice because it reduces swelling, but you don't put it directly on your skin. Wrap the ice/ice pack with a cloth if you need to put it to your skin for a long period of time.
