We need the species to help you right, but I am going to guess it is a Red-ear Slider.
Young Red-ears (up to about 4" long) eat almost nothing but meat in the wild. In captivity, we use about 1/2 good quality pellets, and the other 1/2 is a MIX of worms, small fish, krill, shrimp, insects, and other live or frozen/thawed 'fish foods'.
When the turtle is over about 4", we add some water plants or dark, leafy greens (turnpi greens, dandelion, etc.) but NO head lettuce.
Vitamin A problems are not common in water turtles (mostly in mis-fed box turtles). It is more likely an infection or result of stress-either of which is worsened by a bad diet.

No turtle or tortoise can survive on worms alone. They need plant material and vitamins to supply vitamin A. Is it a RES? Go to this website and look up the foods you should be feeding it--diet will change with age.

You didn't identify the species, so nobody here can tell you what's best for it. Each species has its own dietary requirements.
You should read some books on basic turtle care.
Ignore the posters that said to feed it lettuce (I can't understand why people just make something up when they don't know the answer), as most turtles won't eat it. It is nutritionally useless.

Please feed your turtle something other than lettuce as a vegetable. Lettuce has almost not vitamins and is very high in water content, and will cause your turtle to have watery feces. It is also not as filling as some other greens such as collard greens.

Worms ??? Turtles should not eat worms, they like feeder fish, greens like romaine, and they should have a turtle diet like the sticks that float on top of the water, Im suprised he's not dead yet. Read up on an animal before you get one.
