If the hysterectomy included a removal of the ovaries (bilateral ooforectomy) she can have a lack of hormones and her vagina can be too dry and prone to get tears and wounds. She should use estrogen in vaginal cream and talk to a gyn.

Not having sex often can cause the woman to have to stretch out again when she begins having sex again.
If this is not the situation, get to the dr. Even after a hysterectomy, pap test are still required. There is still risk of vagial cancer.
