Official MLB Rule 5.10c(1): If an accident to a runner is such as to prevent him from proceeding to a base to which he is entitled, as on a home run hit out of the playing field, or an award of one or more bases, a substitute runner shall be permitted to complete the play.

This actually happened a couple of years ago to former Red Sox player Gabe Kapler. He hit a shot and wasn't sure if it was gone or not. As he was rounding second he saw the ball just clear the wall and went from a full sprint and tried to slow down. He broke his leg on the turf and couldn't continue, and a pinch runner finished his homerun trot. He was carried off.

Worth noting, the pinch runner is credited the run scored. The injured batter-runner gets everything else.
