The movie 8mm is pretty hard to watch (I still don't know why I did it, guess because I'm a Joaquin Phoenix fan), but the parts where they venture underground to see illegal snuff films is so sad and pathetic and makes your stomach hurt to watch. (I don't think I would watch that movie again)
oh, and the scene in Boys Don't Cry was so hard to watch, I actually cried when it happened.

Ok it happened in the movie The Divinci Code with Tom hanks. It shows a scene where the detective flashes back as a little girl and found her grandpa having sex on the table with a beautiful young girl. While there doing it, he is surrounded and watched by his cult of at least 30 people. Very disturbing scene!!

Easy The Rape Scene from The Film "Deliverance" With The Hicks and Ned Beatty UGGGGGG. Not even The Hills Have Eyes Had the Brass To Go THAT Far.One Of My Favorite Films But I Cannot hardly watch that scene at all it makes my skin crawl.
Honorable Mention
The Scene In "Alien" when the creature chews its way out of John Hurt.Tuff to watch also.

Looks like many of us are disturbed by rape scenes. I echo the sentiments of an earlier answerer about the brutal (and seemingly endless) rape scene in IRREVERSIBLE.
I'd also mention the even more brutal scene later in the same movie when a man has his head visibly pummeled to a pulp with a fire extinguisher. It's impossible to look away, but it's almost too much to watch.

Pretty much ANY scene from any of the SAW movies. Very graphic.
Also the Silence of the Lambs part when the skinned body is hanging up like an animal on display. For some reason, that has haunted me for the last 10 yrs.

Have you ever seen the movie "Fire in the Sky"? If you have then you'll know exactly what scene I'm talking about. You know, the one where the guy is basically being tortured by aliens and they stick a needle in his eye while he's plastered to a table? Yeah that one is very disturbing. Still gives me the heebie jeebies.

I would have to see that the most disturbing scenes I have ever seen have all come from Sin City. That whole movie made me uncomfortable.

The part in The Exorcist where the Devil stabs Regan brutally (basically looking like Regan stabs herself). On top of that, the Devils says a phrase very affending to Christians.
