If they are so good at hiding it, then how do you know about it? Furthermore, unless you are a trained Psychiatrist, you are only foming an opinion of the persons personality based on your own perception of normality.
If you feel like someone may be a hazard to themselves, or to others, you can report this to either the police, councelors, or other officials. Avoid the person, and when they go on a shooting spree you can kick back knowing that the system failed them, and not You.

I agree that if that person is so good at hiding it, how do you know? but it's an illness, you should tell them or notice people around you so that they can help that person too. You should not think them abnormal because it's just an illness. if no one help them so they may do something bad because of losing conscience.
