At my hospital, we have a 24 hour emergency OB that will handle any c-section where there is no other physician available. Other hospitals usually only allow midwives to deliver there if they are associated with a partner OB who could be on call - and might not make it on site quickly.
If you are talking about a home birth, a midwife simply cannot do a c-section, so 911 has to get called, or the mother has to get herself to the nearest hospital. Keep in mind, that a baby is likely to suffer brain damage if it goes for more than 10 minutes with oxygen depravation. Also, a woman who is hemorrhaging can bleed to death in 7 minutes.

My midwives work with one of the OB doctors offices in town. If they have an emergency, or a pregnancy that they deem is too high risk they call them.
I know that there is usually an OB doctor on call and they can respond in a matter of seconds to minutes.
